This is NOT a scam or identity theft. This is the Condition Care program that is offered at NO COST to supplement your Anthem Health Care Insurance. It provides you with access to a Nurse Line you can contact. They will also check in with you periodically. You CAN opt out, but if you do, you won't be able to call their line either. They won't ask for your personal information (other than your year of birth) and they already have some information regarding conditions you may have (based on information from your Anthem claim history). So, if you don't want them to call you, simply accept the call, then tell them you do not agree to be contacted and they will stop calling.

They seem to be giving up on calling ... now they sent emails!! This is what my email said, ""Four three extension four zero four zero four between the hours of eight am and eleven pm Eastern standard Time Monday through Friday and Saturday nine am to six pm Eastern standard Time. So that one of our health professionals can discuss further with you the purpose of our call to avoid future calls such as this please contact us toll free at 8665256243."

First of all I want to say that me this number called several times and never been left on the answering machine message. This makes the number a little less serious. The last call I was able to answer it and told me that I was chosen for a project. I should Syndicate only my data and then it could go. SCAM!
